Job Description

**Job Overview** The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team. **Responsibilities** + Coordinate patient care in both pre- and post-transplant phases + Collaborate with interdisciplinary team members within the organization and outside community partners + Act as a liaison for transplant physicians to implement medical management of conditions + Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes + Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes + Assumes on-call responsibilities after office hours by providing a resource for medical triage. Acts as transplant coordinator when donor organ becomes available. + Provides on-going education to patients and families + Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects + Participate in the development and revision of protocols + Maintain knowledge of current practices in transplantation, nursing, and healthcare **Required Qualifications** + Bachelors Degree in Nursing or + Associates Degree in Nursing + If Bachelors Degree: 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg or + If Associates Degree: 5+ years of acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg + MN Registered Nurse (RN) License + Basic Life Support (American Heart Assoc or Red Cross) **Benefit Overview** Fairview offers a generous benefit package including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
