POSITION SUMMARY: The Medication History Technician helps to optimize the medication history process and decrease the number of medication errors due to incomplete or inaccurate information. Also helps to improve medication safety by assisting pharmacists in collecting up to date medication lists for patients being admitted to the hospital. Also assists in optimizing patient outcomes through obtaining a complete medication history for patients admitted to the health system as well as updating and clarifying allergy information in patients' electronic health record.
EDUCATION/TRAINING: High school education, or a GED; any advanced Pharmacy Technician education is a plus.
LICENSURE/CERTIFICATION: Candidate will hold a current Pharmacy Technician License in good standing with the State of Illinois or be license eligible. Candidates must obtain a certification through a Pharmacy Technician Certification Board by the second year after obtaining their original Pharmacy Tech license, with the exception of Pharmacy Students who are exempt since they will become pharmacists. This requirement shall follow the rules set by the IL Board of Pharmacy.
EXPERIENCE/SKILLS/ABILITIES :
Hospital and Outpatient Pharmacy knowledge, including, drug name knowledge, dosage forms, and indication for use. Additional training in customer service skills, communication skills, and various software
