JOB RESPONSIBILITY
ASSESSMENT
Head-to-toe skin assessment done bi-weekly only on the weekend you
cover
If the resident has a wound, assess and measure weekly
Monthly weights must be monitored and recorded on MAR by nurses
For residents who need circumference measurements, one nurse should be
assigned for measurements
CHART
The deceased resident’s CHART must be broken down and stored within 24
hours of passing
All narcotics for the deceased resident should be destroyed, and regular medication
Return to the Pharmacy for the resident’s credit
All nurses are responsible for the destruction of NARCOTICS ( residents
order changed / deceased)
Check MARs for holes for the last seven days. Do this on your weekend hours
duty
Nurses are to ensure all re-ordering of medication is up to date ( no less
than 15 days’ worth of VA and Sidney cancer meds and 3 days’ worth of all
other regular med.)
Nurses are responsible for monthly MAR checks to ensure correct
medication administration.

Schedule in facility time around the CNA schedule so you may assist with the pass
med
TREATMENT
All residents’ treatments must have an order, must have notations, etc.
- Oxygen must have dosage time to administer written on MAR
- Nurses must ensure the concentrator is working properly
- Cords are properly connected
- If the concentration system uses a water bottle, it is in place a
contains distilled water
- Tubing is dated and must be changed monthly
Transportation scheduling for all residents’ appointments is done by nurses

ADMISSION
All nurses must ensure that an order to admit is on file for all residents at
admission
Nurses must follow up with the resident’s PCP for a date and time of the first visit
with resident
Nurses to ensure TB test step 1 is administered upon admission, step 2 in 7
days
Nurses must complete a full assessment on admission or upon returning from the
hospital for a prolonged period
72-hour admission follow-up must be completed on all new residents
A 72-hour follow-up was conducted on all residents
ADMINISTRATION
Nurses will be responsible for administering (or recommending to
administrator) disciplinary action against a CMA or CNA who fails to
adhere to the standard of care of each house.
Nurses are responsible for holding staff accountable for delivering high-quality
of care, will we continue to provide outstanding care as we remain
compliant with state and federal regulations.
The nurse will perform any administrative duty as requested by the employer
which is critical at the time for the smooth functioning of the company.
