Key Responsibilities:
Provide therapy and social work services to individuals and families in a supportive setting
Conduct thorough assessments to understand the psycho-social needs of patients
Develop and implement personalized care plans that align with patients' goals and needs
Collaborate with a multidisciplinary team of healthcare professionals to ensure holistic patient care
Assist patients and their families in connecting to community resources and support services
Offer counseling and emotional support to patients and families during challenging times
Advocate for patients' rights, ensuring they receive the best possible care
Maintain accurate and up-to-date patient records and documentation
Provide supervision and guidance to limited license social workers (LMSWs), tracking their progress
Qualifications:
Master's degree in Social Work, Psychology, Counseling, or a related field
Licensed therapist in Michigan (preferred)
Previous experience as a Medical Social Worker is a plus
Knowledge of medical coding and documentation best practices
Excellent communication and interpersonal skills
A passion for delivering high-quality care and supporting individuals and families
Ability to work independently and manage your own schedule
