The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients. Duties include but are not limited to:
· Identifies patients with barriers to a safe discharge plan.
· Completes psychosocial assessments on all patients.
· Documents all clinical assessments, notes, and referrals in Epic.
· Completes screens and coordinates all new skilled nursing facility and rehab facility referrals.
· Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault.
· Educates patient/family about post-acute care options and the role of choice.
· Educates patient/family about managed care and payor issues
This is a full-time day shift position 9:00am-5:00pm Monday-Friday including every 4th weekend, both Saturday and Sunday, located at NewYork-Presbyterian Brooklyn Methodist Hospital.
Preferred Criteria
At least one year of experience in an acute care setting
Discharge planning experience
Experience with Epic and Allscripts
Requirements:
Master’s Degree in Social Work (MSW)
New York State License as a Licensed Masters Social Worker (LMSW).
Communication and negotiation skills
Ability to work independently and collaboratively with the interdisciplinary team.
Work efficiently and effectively under pressure.
Work comfortably within the limitations of the medical setting.
Computer literacy.
