The MDS Coordinator (LPN or RN) is responsible for ensuring patients receive proper care by completing the Resident Assessment Instrument and acting as an in-house case manager. This role involves coordination with physicians, families, and facility staff to optimize resident care in compliance with regulations. Candidates must hold a current nursing license and have clinical experience, particularly in long-term care settings.
