Licensed Practical Nurse (LPN) – Community-Focused Primary Care

Location: Poughkeepsie, NY

Job Type: Contract-to-Hire

Schedule: Monday–Friday, with 1–2 evenings per week (late start on those days)

• Monitor and document vital signs, patient measurements, and clinical observations.
• Prepare exam rooms and manage medical supplies to ensure a safe, organized, and hygienic environment.
• Assist with patient intake, follow-up care, and education.
• Maintain accurate and timely documentation in Electronic Medical Records (EMR) systems.
• Collaborate with clinicians to anticipate patient needs and support care delivery.
• Serve as a resource for nursing-related questions and potentially mentor other LPNs.
• Coordinate patient scheduling and medical testing logistics.

• Licensure: Active LPN license in the state of New York.
• Experience: 1–2 years in a medical office setting; 2+ years preferred.
• Skills: Strong clinical judgment, patient care abilities, and reliability.
• Technology: Experience with EMR systems; EPIC preferred.
• Certifications: Basic Life Support (BLS) certification is a plus.

Pay and Benefits

The pay range for this position is $28.00 - $35.00/hr.
