The Home Therapy Registered Professional Nurse performs all procedures of dialysis and related patient care and participates in the training program for home dialysis and self-care under the supervision of the Director of Nursing.

The Home Therapy Registered Professional Nurse provides chronic dialysis care and treatment, training and education; evaluates patients, communicates with physician and other members of the care team to interpret, adjust, and complete patient care plans; teaches skills, techniques, and procedures; and assists in research studies.

The Home Registered Nurse is responsible for assisting and supporting the unit management team in achieving quality, patient satisfaction, staff development, business management, teamwork, and culture and employee satisfaction.

The Home Registered Nurse will be responsible for the performance of all of the duties listed. The distribution of effort between the duties listed can be expected to vary according to the need and to individual assignments, under the supervision of a Dialysis Manager and Physician Director:

• Demonstrates applicable knowledge of policies and procedures related to the Home Dialysis Registered Nurse
• Demonstrates ability to perform technical skills for the Home Dialysis Nurse including patient’s treatment, and emergency procedures, as determined by the competency assessment program.
• Recognizes and utilizes opportunities for patient and family education and training.
• Participates in teaching programs and provides opportunities for new patients to learn dialysis skills, techniques, and theory.
• Demonstrates applicable knowledge of adult learning theories and incorporates adult learning principles into nursing practice.
• Periodically re-evaluates all home patient’s procedures and updates their training as needed.
• Maintains accurate up-to-date training records.
• Documents clearly, completely, and accurately all pertinent information on patient record.
• Maintains confidentiality.
• Participates in the formulation, update, revision of nursing care plans; including long and short term goals.
• Assumes administrative duties under the direction of the Home Dialysis Coordinator.
• Communicates effectively with physicians, supervisors, co-workers, patients and families.
• Maintains positive working relationship with Medical Director/Medical Staff.
• Performs duties in any area necessary according to ability and experience to keep the unit functioning as efficiently as possible.

MINIMUM QUALIFICATIONS

• Graduate from an accredited school of nursing. Bachelors in nursing or health related field preferred.
• One year RN experience.
• Current, unrestricted RN license in the state of KY
• Hemodialysis/Home PD/HHD experience preferred
• ICU/CCU experience preferred.
• Successful completion of training course in theory and practice of hemodialysis & Peritoneal dialysis preferred
• Valid driver’s license and insurance, if travel involves driving an automobile.
• Computer knowledge desirable.
• Successful completion of current CPR Healthcare Recognition according to the American Heart Association
