Comprehensive Support Services for Adults in Neurological Recovery and Daily Living

• Post-Hospitalization Care & Recovery Support
  Provide personalized care plans to support adults transitioning from hospital to home, focusing on recovery goals, rehabilitation coordination, mobility assistance, and continuity of care to reduce readmission risks.
• Personal Care & Assistance with Activities of Daily Living (ADLs)
  Deliver respectful and dignified support with daily personal care needs such as bathing, grooming, dressing, toileting, mobility, and hygiene routines tailored to individual preferences and capabilities.
• Relationship Building & Promotion of Independence
  Foster positive interpersonal relationships through active listening, empathy, and encouragement, while promoting client autonomy, self-determination, and skill development for independent living.
• Medication Administration & Health Monitoring
  Ensure accurate and timely administration of prescribed medications, monitor vital signs and health status, and coordinate with healthcare professionals to support medical compliance and early intervention.
• Nutritional Support: Meal Planning, Preparation & Dietary Management
  Prepare balanced meals in accordance with dietary restrictions, cultural preferences, and medical needs, while promoting nutrition education and supporting independent or assisted meal preparation.
• Community Integration & Transportation Services
  Offer transportation and support for participation in social, recreational, educational, and vocational community activities, enhancing quality of life and social inclusion.

Job Requirements

• High School Diploma or equivalent
• Valid State of Michigan driver’s license
• Ability to lift 50 lbs
• Ability to pass background checks as applicable
• Ability to become certified in CPR/First Aid
• Possess basic computer skills

Available Shifts:

• 1st Shift: 7am - 3pm
• 2nd Shift: 3pm - 11pm
• 3rd Shift: 11pm - 7am
• Every other weekend availability is required
