Come and join our Diabetes Education team. We are looking for a compassionate RN CDCES, or an RN with a passion for diabetes who is looking to become a Certified Diabetes Care and Education Specialist, to help patients live well with diabetes. Our education department helps people with or at risk for diabetes, gain the knowledge and skills needed to change behavior and successfully self-manage diabetes and its related conditions. The team is committed to improving the health of the community with education, coaching and support for our patients with diabetes. · Collaborate with physicians in the coordination of care of the patient.
· Serve as a resource to nursing staff as well as to other health care personnel involved in the care of the specific specialty.
· Responsible for the development and ongoing coordination of care of the patient in the specific specialty.
· Glucose reviews, insulin and medication management, assess patterns.
· Scheduling patients, phone calls, troubleshoot meters and pumps.
· Consult with individuals, families, community agencies, or colleagues from associated disciplines or within nursing.
· Serve as a resource in educating the patients, family and community regarding information relevant to health and wellness and illness prevention.
