Job Description - Advanced Practice Provider Medical House (250006FZ) Advanced Practice Provider Medical House - ( 250006FZ ) Seeking an Advanced Practice Provider for the Medical House role at UH St. John's Medical Center. The APP will be responsible for hospital admissions, general house calls, care coordination, and discharge planning in collaboration with a multidisciplinary team. The APP will rotate to cover the Rapid Observation Unit. The schedule is primarily day shifts with occasional evenings, weekends, and holidays. APP experience is preferred, but new grads may be considered for the New Grad APP Training Program. Overview The Advanced Practice Provider is responsible for delivering quality patient care, enhancing access, and improving patient satisfaction. Tasks are performed according to the Ohio State Board of Medicine or Nursing and UH Medical Bylaws. Responsibilities Build and maintain positive relationships with patients and customers (10%) Maintain positive relationships with co-workers (10%) Support self-development and foster a learning environment (10%) Align with the hospital's mission and values (10%) Perform patient assessments and determine diagnoses based on data and current research (20%) Develop care plans and implement interventions per care protocols and standards (15%) Collaborate with healthcare team members for patient and family care (5%) Contribute to staff, patient, family, and self-education (5%) Demonstrate leadership and management skills (5%) Provide educational support through modeling and in-services (5%) Engage in professional development via self-learning and peer review (5%) Additional Duties Perform other assigned duties Follow all policies and standards Refer to departmental documentation during orientation Maintain confidentiality and security of Protected Health Information (PHI), including annual training and compliance with UH policies Education Master's Degree from an accredited PA program (Required) Master's Degree in Nursing (MSN) (Required) Work Experience At least 1 year as an APP (Preferred) Knowledge, Skills, & Abilities Leadership skills (Required proficiency) Interpersonal skills (Required proficiency) Computer skills for accessing medical data (Required proficiency) Proficiency in Microsoft Office and Outlook (Required) Licenses and Certifications Physician Assistant (PA) license (Required) Basic Life Support (BLS) certification (Required) Advanced Cardiac Life Support (ACLS) certification (Preferred) Pass the Physician Assistant National Certifying Examination (Required) Supervision by a physician (Required upon hire) Maintain PA certification (Required) Ohio and/or Multi-State RN license (Required) Basic Cardiac Life Support (BCLS) (Required) Advanced Cardiac Life Support (ACLS) (Required) #J-18808-Ljbffr University Hospitals
